Anthem Balanced Funding is open for Georgia Small Group business

On November 1, 2024, we will begin accepting requests for quotes on Anthem Balanced Funding (ABF), our new funding solution for employers with 20 to 50 enrolled employees. Effective dates will begin on January 1, 2025.

  • Current and renewal rates, including rate component breakouts for groups that are currently self-funded, such as admin, rebate credits, individual stop-loss premium, aggregate stop-loss premium, and/or claim factors.
  • Plan designs for all current and renewal plans. Summary plan documents are preferable to a summary of benefits and coverage (SOB).
  • Census information, including first and last name, ZIP code, gender, date of birth, coverage tier, plan selection, and distinguishing labels for employees and dependents.
  • Claims experience for the most recent 12- to 24-month period, including both claims and enrollment by month.
  • Large claim information, including dollar amount and diagnosis for the same period as the claims experience.
  • Competitive quotes already received.
