The law requires us to confirm your identity before disclosing PHI

To remain good stewards of your client’s information, we are legally required to confirm your identity before providing any protected health information (PHI) about them. We do that by asking for your full name, then three identifiers for your client, such as name, date of birth, and Health Care Identification (HCID) or Member ID. If you do not have them, we may accept alternate identifiers, like a Social Security Number or full address.

Please be sure you have identifying information ready when you contact Broker and Employer Services. To protect the members’ PHI, we cannot provide that information to you.
