New and existing Southern Ohio Chamber Alliance Benefit Plan groups are eligible for even more savings when they add new dental or vision coverage. These increased credits vary by group size (based on number of enrolled medical subscribers). For example, a group with seven enrolled medical subscribers would receive a one-time $2,000 credit to their medical invoice when adding dental and vision.
* Discounts are applied to medical premium equivalent rates and issued as a one-time credit within 90 days following the effective date. Credit is subject to underwriting approval based on dental and/or vision products purchased and the determination of final medical enrollment. Anthem’s credit will remain the same regardless of the actual expenses the group may incur. Credit only applies in the first year when new Anthem dental and/or vision coverage is purchased either in combination with a new or added to an existing SOCA Benefit Plan medical plan. Excludes voluntary products. Other restrictions and limitations may apply. Please contact your Anthem representative for more information. Discounts apply to new Anthem medical MEWA groups and converting or existing medical MEWA customers who purchase new Anthem employer-sponsored dental and/or vision coverage. Credit only applies to dental and groups with 2 to 50 enrolled. Other restrictions and state-specific limitations may apply. Please contact your representative for more information. We reserve the right to modify or terminate this offer at any time.
