The CaliforniaCare HMO agreement between Anthem Blue Cross and Affinity Medical Group will terminate effective September 1, 2025. As a result, the Affinity medical groups listed below will no longer be part of the commercial Anthem Blue Cross HMO Network beginning September 1, 2025.
- Affinity Medical Group-Alameda Region (KB9)
- Affinity Medical Group-Bay Valley Region (KCI)
- Affinity Medical Group-Eden/San Leandro Region (KCL)
- Affinity Medical Group-Rossmoor Region (KCH)
- Affinity Medical Group-South Bay (KCJ)
- Affinity Medical Group-West County Region (KCK)
Every effort is being made to ensure that your clients continue to have access to their current primary care physicians and specialty providers through alternate medical group affiliations.
Clients affected by this change are being notified that as of September 1, 2025, Anthem will assign them to an alternate medical group. New member ID cards will be mailed no later than five days before their new effective date. They can also use the Find Care feature at www.anthem.com/ca to find available providers.
Please let your clients know that while their medical group may change, their benefits remain unchanged.
Your clients should contact an Anthem customer service representative using the toll-free customer service number, 1-844-971-0117, or the toll-free member services number on their current ID card, if they have concerns about their care.
