Clients can earn an additional $25 reward by completing a Health Assessment. This online tool offers meaningful insights into their overall health and helps support more personalized care decisions.
Highlight year-round flexibility
While certain rewards are time-based, remind clients that wellness visits and Health Assessments can be completed anytime throughout the year, giving them flexibility to engage when it works best for them.

Guide your clients to log in to anthem.com or the Sydney® Health app to get started. You can also share this member flyer about the incentives program so they can learn more.
*Colorado HMO is not eligible for wellness incentive. Enhanced Limited Duration plans in Georgia, Indiana, Missouri, and Wisconsin are not eligible for an incentive.
