Appointment Needed for Access to Battle Born Plan Clients

Brokers who wrote Battle Born plans and need ongoing access to support their clients must request an appointment with Community Care Health Plan of Nevada. This appointment is required to ensure proper access to client information and to continue providing service and support for these plans.

If you have not yet completed this step, please do so as soon as possible to avoid any disruption to client access.

To begin the appointment process, log in to Producer Toolbox and locate the “Appointment” paperwork. The required forms can be completed and submitted online directly through the portal.
