We are expanding our Small Group Anthem Balanced Funding portfolio with the introduction of four new health plan options with a July 1, 2025, effective date. These plans are designed to serve the Small Group market, catering to businesses with 20 to 50 enrolled employees.
As part of Anthem's Balanced Funding offering, these new options provide your clients with more flexibility and control.
There will be two plans on the Blue Open Access point of sale (POS) network and two plans on the Blue Connection exclusive provider organization (EPO) network.

Anthem Balanced Funding Blue Open Access POS network:
- Deductible $3,000/Coinsurance 20%/Out of pocket maximum $8,500
Anthem Balanced Funding Blue Open Access POS network:
- Deductible $500/Coinsurance 20%/Out of pocket maximum $4,000
Anthem Balanced Funding Blue Connection EPO network:
- Deductible $3,000/Coinsurance 20%/ Out of pocket maximum $8,500
Anthem Balanced Funding Blue Connection EPO network:
- Deductible $500/Coinsurance 20%/Out of pocket maximum $4,000
