For the 2025 tax filing year, Anthem is here to assist your Georgia FARM and SMART plan clients. These groups are required to annually submit 1094-B and 1095-B forms, and we're making the process easier.
Anthem will prepare the necessary 1094-B and 1095-B forms and upload them to the EmployerAccess portal.
The Georgia Chamber of Commerce and the Georgia Farm Bureau, through their health plan management provider, Consoliplex, will file these forms electronically with the IRS for all involved employer groups by March 31, 2026. Take a look at the SMART and FARM fliers for more information.
