Our pregnancy-care program, Future Moms, is changing to Building Healthy Families (Upstate / Downstate), for most Large Group ASO, JAA, and National groups on January 1, 2023. It will launch for other groups in 2024.
The definition of a family has changed since Future Moms launched many years ago. Building Healthy Families is designed to allow your clients to be more responsive to a wider array of employee needs and provide better support for returning to work. Building Healthy Families is an end-to-end digital solution that goes beyond traditional maternity support to offer helpful resources from preconception through early parenthood. Members will have 24/7 access to the program through the SydneySM Health mobile app, Engage, or our website. In addition, pregnant members can receive personalized guidance from Family Care Coaches and nurses through digital chat and telephone support. It was built to meet the needs of today’s modern workforce and is powered by AI to create a personalized experience.
Other key features:
- New digital experience – offering employees easy access to a library of family planning content and tools for preconception, maternity, pediatrics, parenthood, and return to work.
- Increased care – ongoing risk assessments, personalized care plans, monitoring, and clinical escalation pathways to maternity nurses.
- Next Best Actions – offers reminders to pregnant members providing ongoing pregnancy risk monitoring with proactive intervention for high-risk pregnancies, personalized resources, and suggestions.
- Culturally responsive and inclusive - recognizes and supports a diverse population of members such as families of color, the LGBTQIA+ community, and those impacted by social determinants of health.
