New Small Group Specialty Benefit Modification Form for August 1 and later effective dates

Starting with August 1, 2024, effective dates, you must use our new 2024 Specialty Benefit Modification Form for Small Groups in California. The Life & Disability sections have been removed now that our partner, The Standard, has completed its purchase of that business.

You can download the new benefit modification form or find it in Producer Toolbox. Please begin using this updated form now. The old version will not be accepted for August 1, 2024, or later effective dates. This also applies to all specialty add-on and upsell opportunities.
